Renders lowercase letters as small capitals.

Remarks

Two-tier approach: Native: activates OpenType 'smcp' feature via HarfBuzz (proper small cap glyphs). Synthesis: converts lowercase to uppercase and scales down (fallback for fonts without smcp). Attribute byte encoding: 0 = unchanged, 1 = native smcp, 2 = synthesis.
